About Your Visit
Below, please find some information that may be helpful in preparation for your visit with MRH.
Your First Appointment
Because Dr. Herzog is a specialist, a referral from your primary care provider is often required. Prior to your visit we will mail you the necessary intake paperwork. Please take the time to thoroughly read and complete this paperwork and bring it with you to your first appointment. Completion of these forms will allow us to better serve you.
If your appointment is for a work related injury, please make sure that you include your workers compensation insurance carrier’s name, your adjuster’s name and phone number, your claim number, date of injury and employer’s name. We will need this information to obtain the necessary pre-approval for any additional treatment, tests, x-rays, MRI’s etc., as well as for billing. Additionally, please ensure that your medical records and other relevant information (x-rays, MRI’s, etc.) are received by our office prior to your appointment date. Your primary care provider or other specialists can assist you with this.
If your appointment is for an injury from a motor vehicle accident, we will need to know this prior to your visit. You must complete all information for both drivers to include insurance company names, including addresses and telephone numbers, claim numbers, date of accident and the adjuster’s name. If you are represented by an attorney, we will also need his/her name and telephone number. All intake paperwork must be signed at the time of your visit.

As a courtesy we call to remind you of all appointments; however you are responsible for the time that is reserved. When you do not keep your scheduled appointment it takes time away that could be offered to other patients in need of care. In order to accommodate all patients we require 24 hours notice of cancellation. For less than 24 hours notice and failure to show for these appointments our policy is to charge a fee which must be paid prior to rescheduling.
As many of our patients and staff have chemical sensitivities, we ask you to be considerate of this and refrain from wearing scented products such as perfumes, lotions, after shaves, etc. when you come to your appointment. Your understanding is much appreciated.
What To Expect With Treatment
If you have any questions about services provided or individual treatment responses, feel free to discuss this with the staff.
MRH accepts most major insurance carriers. Please contact your insurance company if you have any questions regarding network participation. If your insurance plan is an HMO, please ensure that any necessary insurance referrals are authorized by your primary care physician prior to your visit. Please bring your insurance card with you to your appointment. Co-payments are required at the time of service. Please call ahead to discuss other possible payment arrangements with the office staff.
